Resources for Primary Care Providers
To encourage healthy eating and physical activity and prevent overweight, Nemours Health & Prevention Services (NHPS) is working with primary care pediatric offices in Delaware to provide tools, educational materials, and guidelines for health care providers, parents, and children.
We are working to provide tools and resources for providers, patients, and their families, including clinical management guidelines and take-home information for parents and children.
Primary Care Provider Toolkit
NHPS has created a toolkit to help you identify and manage overweight in the children you see at your clinic. The toolkit includes the following childhood overweight references for primary care providers:
- BMI charts for boys and girls ages 2-20
- BMI wheel
- Guidelines for prevention, identification, and management of overweight in children 2-18 years
- Guidelines for prevention, identification, and management of overweight
in children birth to 2 years - Guidelines for the medical evaluation of overweight children ages 2-18
- Indications for subspecialty referral
- Weight goals
- Blood pressure level charts for boys and girls by age and height percentile
- Survey for all patients and families at well child visits (in both English and Spanish)
