Thank you for using the e-Referral on Nemours.org. Please read the following instructions carefully.

This application is not to be used for emergencies or to request an urgent appointment. All medical emergencies should be handled by calling 911 or your local emergency access number.

• All referrals are assigned a tracking number once they are entered into the system. The tracking number will be displayed upon final confirmation. Please print this information for your records. If you have not reached a screen that displays a tracking number, your referral has NOT been entered into the system.
• After your initial agreement, you will not be taken to this screen by default; however, if you wish, you will be able to view this page by selecting the "View Instructions" link at the end of the e-Referral form.
• You are encouraged to check on the status of past referrals you have submitted. Contact the office located on your confirmation sheet for the status of the referral and pertinent information about the referral.
• If you experience difficulties using this referral system please contact the Nemours Help Desk.
