Division of Transition of Care
By their 21st birthday, all patients treated at Nemours/Alfred I. duPont Hospital for Children in Wilmington, DE, must transition to adult health care. Since there are many transitions that occur between the ages of 18 and 25, families often aren't adequately prepared.
Our Transition of Care program is a clinical consultation service designed to help teenage and young adult patients (particularly those with special health care needs) and their families transition as smoothly as possible from pediatrics into adult health care. We address both the medical and non-medical issues during transition (educational, vocational, legal guardianship, residential options, etc).
Ideally, patients over the age of 14 with special health care needs will develop transition plans with their providers and parents. The patients who typically
benefit most from working with our Transition of Care team are kids who:
- see multiple subspecialists in pediatrics and, therefore,
may have a complicated transition - have disabilities that will make it difficult to be completely independent as adults
A senior pediatrician and social coordinator run our Transition of Care program. Together, we work with young adults and their families to create transition plans. To gauge progress, we meet with the family and child on a regular basis.
Transitioning care involves a lot of preparation and is a process, not just a transfer. A great place to start is by finding ways to increase independence or co-manage medical issues. To assist teens in beginning the process of developing a transition of care plan with their providers and parents, we help:
- identify adult health care providers who are knowledgeable about the child’s medical condition and with whom the family/child are comfortable
- kids learn more about their medical conditions
- teens and young adults manage their own care and become advocates of their own health (e.g., how to refill prescriptions and talk to adult providers)
- families work with insurance providers
- kids and their families learn about residential options, especially for patients who have disabilities that don’t allow them to function completely independently

The Healthcare Information and Management Systems Society (HIMSS) awarded duPont Hospital for Children the Stage 7 Award.
Stage 7 represents achievement of the highest level on the EMR Adoption ModelSM (EMRAM) – used to track electronic medical record progress at hospitals and health systems. Nemours is one of 59 (or 1.1%) of the more than 5,000 U.S. hospitals in the HIMSS Analytics Database recognized by HIMSS Analytics with its Stage 7 Award.
Nemours also received the HIMSS Davies Award of Excellence for
helping pioneer the use of electronic medical records in pediatric health.
The award recognizes leadership in the implementation and use of health information technology to improve patient care and medical outcomes.
