Steps for applying for a pediatric sub-internship or subspecialty elective (LINK here to electives available)

1. Complete online application. 
2. E-mail or fax application to Cindy Chuidian at cchuidia@nemours.org or 302.651-5954. You may also phone her with questions at 302-651-5795. You will receive approval via e-mail.
3. When your rotation is approved you will be asked to complete and fax the following materials immediately. Processing of background checks can take up to 6 weeks so please plan accordingly.
   • Child Abuse Background
   • Adult Abuse Background
   • Authorization for Background Check
   • Service Letter Release Form

and provide proof of immunization.

A Quest Lab form will be mailed to your home address to obtain a urine drug screen. This can be done at any Quest lab in the country. For a listing of locations please visit www.questdiagnostics.com or call 800-377-8448. Please note that not all sites perform drug screens. You may also schedule an appointment that is convenient for you.

Please visit www.questdiagnostics.com/psc to do so. This will save you valuable waiting time! Please have the urine drug screen done no later than one week prior to your start date.
