Cardiac Goals and Objectives
Medical Knowledge Focus
Understand the pathophysiology and clinical presentation of:
- Acyanotic and cyanotic congenital heart lesions
- Acquired pediatric cardiac lesions
- Congenital and acquired pediatric dysrhythmias
- Resident is to prepare for cases by researching relevant cardiac issue and have OR or catheterization suite technically set and ready for procedure.
- Resident is to fully discuss case with Attending prior to procedure
Patient Care Focus
The clinical objectives are:
- Gain exposure and understand essential principles of the medical and surgical management of pediatric cardiac lesions, including acute post-operative management.
- Gain exposure and familiarity with the anesthetic management for surgical repair of cyanotic and acyanotic congenital heart lesions
- Gain familiarity of essential principles of pediatric cardiopulmonary bypass and hypothermic arrest
- Gain exposure and familiarity with anesthetic management of pediatric cardiac catheterization and electrophysiological procedures; diagnostic and therapeutic.
- Demonstrate proper performance of cardiopulmonary resuscitation in children
- Resident is to be certified in Pediatric Advanced Life Support
- Demonstrate ability to initiate proper fluid resuscitation
- Demonstrate ability to utilize pharmacologic agents in support of the circulation
- Demonstrate ability to utilize electrical therapy for malignant dysrhythmias
- Demonstrate proficiency in the placement of venous and arterial catheters, including central venous catheters
Practice-Based Learning and Improvement
The Cardiac Anesthesia rotation is not meant as a training arena for a Pediatric Cardiac specialist. Instead, the objective is to gain sufficient exposure and understanding of pediatric cardiac physiology and pathophysiology, along with essential medical and surgical management principles, to enable the pediatric anesthesiology fellow to competently function as an anesthesiology consultant for a child with cardiac pathophysiology having a non-cardiac surgical procedure.
Systems-Based Practice
- Gain familiarity with a specialized pediatric cardiac unit with specialized expertise for care of highly complex patients including:
- Organization of units (OR, CCU, cardiac catheterization lab)
- Triage of patients from intensive care to step down units
- Method of referral and patient transport
Professionalism
- Abide by code of conduct of Nemours Cardiac Center at all times
- Be respectful to patients, families, and staff
- Attend and be prompt to all assigned OR and Cardiac Unit patient obligations
