These reports and lists are formatted from documents used within our organization to help us expedite our referral and authorization processes. They have been modified to be user-friendly to our partners in the community. The information contained is not intended to replace information provided directly to you from any managed care organization, but to compile the identifying numbers for our hospital and specialty care physicians that are required by the individual plans.
