Pre-Registration

Please call our pre-registration office two weeks prior to your child’s surgery.  When calling us, have the following available:

• any medications your child is currently on
• insurance cards
• social security number
• proof of guardianship, if guardianship is with anyone other than the biological or adoptive parent

Pre-Admission Screening

You will receive a call prior to surgery to review your child’s medical history.  The nurse will answer any questions and discuss the events that will occur on the day of surgery.
