Waiver: Permission from the IRB to eliminate completely or alter the requirement for either the parental permission or informed consent process or documentation of the process. Waiver applies to:

  1. Exclusion of any required elements of the parental permission and / or informed consent process.
  2. Alterations that effect a participant’s autonomy, such as research that includes plans to:
  • Intentionally deceive, or misinform participants, or conceal aspects of the research design from the participants.
  • Enroll participants based on presumed consent or “passive consent”, e.g., in the absence of an objection.
  • Defer permission or consent. E.g., after participation begins.
  • Departures from the IRB templates.
  • Permission obtained by telephone.
  • Use of the short form for non-English speaking participants.

Ward: A child who is placed in the legal custody of the State or other agency, institution, or entity, consistent with applicable Federal, State, or local law. (HSP#071)
