Expert Assistance from Your KidsTRACK Team

KidsTRACK (Teaching, Research, Advocacy, Community, Knowledge) is your one-stop source for assistance and access to information and services that can help make your child’s medical experience more manageable. Serving as a hub, KidsTRACK connects you to social workers, financial counselors, spiritual care providers, Child Life specialists, language interpreters, educators, and other hospital services when you need them most.

Collaborating with your child’s medical care team, your KidsTRACK team helps you:
  • coordinate multiple doctor appointments here or at our satellite clinics
  • interact/connect with nonprofits and insurance companies
  • learn to use special equipment for home health care
  • learn to cook meals for a child’s special diet (in the teach-in kitchen)
  • find reliable information on medical conditions, research, and treatments
  • access emotional support for your child and family

Care Coordination

As a Nemours patient with complex medical needs, your child is assigned a personal care coordinator who is a member of your clinical team. Your KidsTRACK care coordinator is with you every step of the way, from hospital life to home life.

Life at Nemours

Your KidsTRACK care coordinator is a social worker who actively works with your clinical health care team, including nursing case managers in our subspecialty areas, Child Life specialists, and spiritual care providers to identify and meet your child’s special needs.

Your care coordinator can:
  • help you understand a new diagnosis, medical equipment, or
    new medications
  • facilitate communication and collaboration among specialists
  • foster open communication with your child’s health care team
  • manage quality of care concerns you may have
  • help you cope with the emotional effects an illness have on your family
  • answer questions regarding your Medicaid rights and the appeals process

Help for Your Child's Transition Home

Kids with complex health conditions sometimes have special needs at home, at school, and in the community. Your KidsTRACK coordinator collaborates with your care team to help your child successfully adjust to life. Whether it’s filling out applications for assistance, scheduling appointments, or identifying support groups, it’s our job make your life easier.

Your care coordinator can:
  • assist in coordinating discharge and related needs like home medical services and equipment
  • help you find and access available federal and state assistance programs
  • provide referrals to health care agencies and community resources (rehabilitative, vocational, and early intervention services)
  • schedule hands-on learning opportunities like food preparation demonstrations for your child’s special diet in our teach-in kitchen
  • connect you to members of our Advisory Councils who have experienced similar health care situations
  • coordinate follow-up and ongoing outpatient pediatric specialty care appointments here at the hospital, or closer to home at one of our area satellite clinics in downtown Orlando, Lake Mary, or Viera.

Education and Group Offerings

In addition to providing our patients and families with vital care coordination services, Nemours KidsTRACK offers educational resources, learning opportunities, and demonstrations.

 
Small Group Meetings & Classes

KidsTRACK features ample conference space for community meetings, special guest visits, and group training.

Scheduled events may include:
  • care provider CPR classes
  • parent/baby classes (breastfeeding, etc.)
  • coping and support groups
  • safety and intervention programs
  • training for medical equipment your child may need at home

Hands-On Learning in the Teach-In Kitchen

Our KidsTRACK team can coordinate hands-on learning in the well-equipped teach-in kitchen. Working closely with other departments, such as dietary, endocrinology, and Florida Prevention Initiative, our chef offers healthy, interactive cooking demonstrations and nutritional education programs for the family's special dietary needs.

Medical Demonstrations & Instruction

If your child is having surgery, our Child Life specialists, anesthesia team, and support staff can prepare your child and your family, with a pre-operation tour. Using real and pretend medical equipment, the team will demonstrate the procedure and answer all of your questions or concerns to help ease anxiety.

Early Literacy Screening

Nemours BrightStart! early literacy programs help identify and teach children at risk for reading failure. Located in KidsTRACK for easy community access, this program offers tools, resources, services, and training to build necessary skills for reading success in children.
