Overhead Code BLUE (Off Unit)

• Two pre-assigned nurses will respond when the overhead announcement of a Code BLUE is made for the second or third floors or outside including the parking garage. The Charge Nurse cell phone will also be paged.
  • The Emergency Department will respond to Code BLUE in all other areas of the hospital (first floor, ground floor.)
  • If assistance is needed from the PICU Code BLUE response team, an overhead page will state "PICU response team is required at (Location.)"
• Bring the portable monitor/ defibrillator/pacer, intraosseous cannula device and the emergency bag from the PICU to the scene. This nurse should remain at the scene to assist with the emergency.
• Assure that first responders are providing effective BLS.
• Participate in the interventions of BLS and ALS appropriate to the circumstances of resuscitation.
• Prepare and administer medications.
• Assure that any additional resources are summoned to the scene of the medical emergency and additional personnel are paged as requested by the directing physician.

In the event of an overhead Trauma Code announcement:

• 1 (One) PICU RN should report immediately to the Emergency Department
• 1 (One) PICU RN should report to the blood bank to retrieve the cooler of blood. Report directly to the Emergency Department to drop off. Assist if required in ED or return to PICU.
  • Blood Process: Signatures are needed at two points in this interaction: 1) the nurse receiving the blood from Blood Bank. 2) The physician in the ED that accepts it.
  • If a blood slip is unavailable from the printer, the PICU RN needs to have a patient identifier (e.g. MRN number) and the Blood Bank will release it.

Overhead Code BLUE

• ED response team will be responding to all Code BLUE called on Ground floor, First Floor, All of ARB and All interstitial ( service) levels
• Code BLUE Team consists of 2 ED techs with a stretcher, back board, C-collar, BVM and AED. Not necessary for crash cart to be at scene.
• Nursing Supervisor responsible for documenting patient’s name and location of Code Blue.
• Patient will be transferred to ED and evaluated.
• If it is determined by responders that additional resources or clinical assistance is needed, call the Command Center at 53-5555 and request the PICU Code BLUE team

• Be aware that adults involved in a Code BLUE event will be transported to the Emergency Department
• The Physician should immediately contact an adult hospital to arrange for a transfer

The AIDHC physician providing intensive care coverage will carry a beeper that will alert the physician to any Code BLUE called within the hospital or hospital grounds.

• When appropriate, delegate interventions of BLS and ALS to properly qualified members of the Code BLUE team

• Serve as a consultative resource to the physician of record or his/her designee in matters related to the conduct of the BLS and ALS efforts
• Provide medical/surgical expertise and consultation
• If the Code BLUE is an adult, proceed immediately to the Emergency Department for further stabilization once safe to transport.
  • Immediately contact adult hospital to coordinate transfer (Christiana or St. Francis Hospital). The accepting facility will provide transport pending an available bed.
• Dismiss unneeded personnel from the scene of the resuscitation. (Support security's role in helping with this responsibility.)
• Physician Director of the Code BLUE team may cancel the Code BLUE, as appropriate

Response to a Code BLUE

• Determine: Name of caller, Location (Room No., floor and area), Type of emergency, and Telephone extension
• Any health care provider in the immediate area will assist in the performance of Basic Life Support until the Code BLUE Team arrives
• NOTE:EARLY DEFIBRILLATION PROTOCOL:
  • An AED shall not be used on anyone under the age of one (1) year
  • Use the AEDs only when the victim has the following three (3) clinical findings
    • No response
    • No breathing
    • No pulse
• Begin resuscitation efforts, including Cardio Pulmonary Resuscitation (CPR)
• Turn on AED and follow instructions for connecting the patient to the device (making note of whether defibrillator pads pre-attached to the AED are Adult or Pediatric and, change, as appropriate)
• The Code BLUE Team members will assume responsibility for the resuscitation effort with appropriate Code BLUE Team members, replacing and dismissing personnel as appropriate; assure space is available for the Code BLUE Team
• The Code BLUE Team members will remain with the patient until either the patient is stabilized or the Code BLUE is terminated by the Attending Physician directing the Code BLUE. Once the patient is stabilized, the Code BLUE physician may instruct the team to transfer responsibility for the patient to the appropriate personnel, which may include relocation of the patient to another inpatient or outpatient care area

• Communicate with the Command Center via two-way radio

Social Work staff will respond to Code BLUE alerts during departmental business hours Monday – Friday, 8:30 a.m. – 10:00 p.m; Saturday and Sunday, 9:30 a.m. – 6:00 p.m. The on-call social worker is available for off hours if end of life and bereavement needs are required.

• Serve as a liaison between the family and the medical team, ensuring that the family is receiving information about their child’s condition and the plan of care in a timely manner
• Secure a private space for the families to sit and wait, or, accompany them to the treatment room when they so desire
• Assist families in contacting family, relatives and others as desired
• If families are observing the Code BLUE, prepare them for what they will see and experience. Remain with family during this observation to answer questions and continually assess emotional status and ability to cope with current situation
• Inform family that you will be available to help them get their questions answered and to arrange meetings with the medical team
• Discuss with families that you will serve as an additional support person to them and can assist them with contacting extended family, friends, clergy, etc
• When necessary, assist families and the health care team through process of making medical decisions for the child, i.e., help them understand brain death protocol, organ donation process, their ability to be with their child throughout the process, etc
• Assist family with identifying funeral home and completing paperwork in Hospital Death Packet
• Offer interventions with patient's siblings as necessary
• If a death has occurred, inform family that you will remain available to them for ongoing support and other bereavement services
• If appropriate, contact the Nursing Supervisor to obtain the bereavement cart

  • Blood Bank personnel are not expected to respond automatically to trauma alerts, unless otherwise notified by a member of the trauma team.
  • The Blood Bank Staff are to report to Blood Bank upon notification of a trauma code. Trauma code response is the highest priority of all activities in the department. All members of the Blood Bank are expected to assist in the many levels of preparation and response.
  • In the event of a trauma code, a representative from the trauma team will call the Blood Bank with 2 unique patient identifiers as soon as they are available. Orders will be immediately placed in the computer and will route to the Blood Bank in basket to be printed. INITIAL RESPONSE TO TRAUMA CODES WILL BE COMPLETELY MANUAL AND WILL NOT INVOLVE BLOOD BANK COMPUTER SYSTEM EXCEPT FOR INITIAL BAD FILE REVIEW.
