Overhead Code BLUE (Off Unit)

  • Two pre-assigned nurses will respond when the overhead announcement of a Code BLUE on floors 1 - 6 including but not limited to OR/IR, hospital grounds, parking garage, discover garden, and clinics. In addition to the overhead page the code blue team will be notified via a personal communication device by clinical logistics.

The RN Procedure Provider Will:

  • Bring to go code bag and an ISTAT machine to the Code Blue event.
  • Insert NG/OG tube as directed by the Code Physician – Team Leader.
  • Obtain labs as directed by the Code Physician – Team Leader.
  • Administer medications under the direction of the Code Physician – Team Leader.
  • Communicate patient data and completed interventions to Recorder.
  • Participate in the interventions of BLS appropriate to the circumstances of resuscitation

The Code Cart RN Will:

  • Prepares medication for administration under the direction of the Code Physician – Team Leader
  • Distribute code cart supplies to Code Blue Team members as needed.
  • Operate and monitor/defibrillator at the direction of the Code Physician – Team Leader.
  • Participate in the interventions of BLS appropriate to the circumstances of resuscitation

The Recorder Will:

  • Communicate with the Team Leader throughout code which interventions have been done
  • Maintain an accurate written record of the timing of all interventions
  • Function as Code BLUE time keeper
  • Complete top portion of the post code debrief form and submits to the Patient Flow Supervisor

  • Dismiss unneeded personnel from area.
  • Assist with directing and coordinating the activities of the nurse participants.
  • Communicate with family
  • Coordinate with family option for presence during code blue event and the option to leave at any point during the event
  • Notify Social Worker, Chaplain, Interpreter, Child Life
  • Assure that any additional resources are summoned to the scene of the medical emergency and additional personnel are paged as requested by the directing physician.
  • Participate in the interventions of BLS appropriate to the circumstances of resuscitation
  • Cancel the Code Blue if instructed to do so by the Code Physician or when the Code Blue has been determined to be a false alarm.
  • Facilitate appropriate inpatient bed placement or out of facility transfer
  • Coordinate the debriefing with follow-up

Specialty Nurses

Emergency Physicians, or their designee, will respond to Code BLUE events on ground floor including but not limited to radiology, MRI, CT, laboratory, and the gym.

  • Call for patient's nurse if s/he is not present
  • Provide overall direction and coordination of the BLS and ALS interventions
  • Participate in those interventions of BLS and ALS appropriate to the circumstances of resuscitation
  • When appropriate, delegate interventions of BLS and ALS to properly qualified members of the Code BLUE team
  • Document in the patient medical record and resuscitation record their participation in the resuscitation as needed
  • Serve as a consultative resource to the physician of record or his/her designee in matters related to the conduct of the BLS and ALS efforts
  • Provide medical/surgical expertise and consultation
  • If the Code BLUE is an adult, proceed immediately to the Emergency Department for further stabilization once safe to transport.
    • Immediately contact adult hospital to coordinate transfer. The accepting facility will provide transport pending an available bed.
  • Dismiss unneeded personnel from the scene of the resuscitation. (Support security's role in helping with this responsibility.)
  • Physician Director of the Code BLUE team may cancel the Code BLUE, as appropriate

 

General Pediatrics

Be aware for over head Code BLUEs anesthesia services may be immediately required for surgery or difficult intubations. The anesthesia team will be readily available and be called via the routine communication systems in the event of a Code BLUE if needed.

The Logistics Center

Emergency Logistic Technician

Code BLUE/Responder 5

If a Code Blue comes through Responder 5 from the inpatient side, clinical logistics takes the lead. The ELT will audio and video into the room and then advise Security to call the code. However, any other area, Security must call a Code Blue immediately. Make sure to confirm the room number with clinical logistics.

However, any other area, Security must call a Code Blue immediately. Make sure confirm the room number with clinical logistics.

  • The Security Command Center makes announcement two times via the overhead fire alarm system “Attention Please …Code Blue (Location: Floor/ Department/ Area with the department)
  • Announce the same informaiont over the “Nemours All” Radio channel
  • Cancel the Code BLUE once instructed to do so by the Patient Flow Supervisor.
  • Announce twice “Attention Please - Code Blue all clear.
  • Capture time 00:00:00 code is called and time 00:00:00 officer arrives on scene using the digital wall clock.

  • Obtain the stretcher and bring to the location of the Code Blue Event.
  • Communicate with the Command Center via two-way radio upon arriving to the area
  • Directs code team members to code location
  • Obtain AED and closest Code Cart at the direction of the Code Blue Team for Code Blue events in non-clinical areas.
  • Direct team members to the resuscitation area, control traffic, and allow only essential personnel into the area.
  • Assist the Code Physician/Patient Flow Supervisor to dismiss personnel from the area as appropriate.
  • Inform the Command Center of the need for additional personnel or
  • Cancel the Code BLUE if instructed to do so by the Patient Flow Supervisor. The Patient Flow Supervisor or the Physician (Code Leader) are the only persons who may cancel a Code BLUE. This is done upon instruction by the Physician Director of the Code BLUE team, or when the Code BLUE has been determined to be a false alarm

    • Blood Bank staff will be notified of trauma alerts via the overhead system. Notification of trauma codes will be via the overhead system and the trauma beeper.
    • The trauma beeper must be audible within the lab or carried by a designated Blood Bank technologist.
    • Blood Bank personnel are not expected to respond automatically to trauma alerts, unless otherwise notified by a member of the trauma team.
    • The Blood Bank Staff are to report to Blood Bank upon notification of a trauma code. Trauma code response is the highest priority of all activities in the department. All members of the Blood Bank are expected to assist in the many levels of preparation and response.
    • In the event of a trauma code, a representative from the trauma team will call the Blood Bank with 2 unique patient identifiers as soon as they are available. Orders will be immediately placed in the computer and will route to the Blood Bank in basket to be printed. INITIAL RESPONSE TO TRAUMA CODES WILL BE COMPLETELY MANUAL AND WILL NOT INVOLVE BLOOD BANK COMPUTER SYSTEM EXCEPT FOR INITIAL BAD FILE REVIEW.
