COVID-19 Screening Information
Important — Before Your Child’s Appointment
To help avoid delays for safety reasons on the day of your child’s appointment, please review the following information.
Answer These COVID-19 (Coronavirus) Screening Questions
IMPORTANT: Please review the screening questions for the state in which you live. If your answer is "yes" to any of them, please call the office where your child is scheduled to be seen to notify us so we can prepare to safely see your child. When calling, tell us you have a “yes” to a COVID-19 screening question.
Screening Questions
1. In the last 10 days, has the patient or anyone attending the appointment had a test showing infection with COVID-19 (a “positive” test)?
2. Is the patient or anyone attending the appointment waiting for results of a test related to symptoms or exposure to COVID-19? Note: Does not include routine testing required for your job, etc.
3. In the last 10 days, has the patient or anyone attending the appointment had ANY of the following:
- Loss of smell or taste
- Persistent cough
- Shortness of breath
- Sore throat
- Fever (children greater than or equal to 100.4 F temperature; adults greater than or equal to 99.5 F temperature)
4. In the last 14 days, has the patient or anyone attending the appointment been in close contact with someone who has COVID-19 or is suspected of having it?
All patients and anyone with them will be screened for COVID-19 when you arrive at your appointment

Bring — and Wear — Face Masks
Face covering is required for everyone 2 years and older entering the building. Masks must cover mouth and nose and be worn at all times. Masks with valves or vents, scarves, bandanas, knitted masks and gaiters are not permitted. If you or your child do not have approved masks, we will provide masks when you come to our office and show you how to wear them properly.
