Requesting Records:

1. How to Request Records/How to request an Accounting of Disclosure
   Authorization to Disclose Form
2. How to Amend a Record
   Request for Amendment Form
3. How to Request a Restriction
   Request for Restriction Form
4. How to Revoke a Restriction: A request in writing will need to be submitted stating that you would like to revoke the authorization. You will need to list the date of the authorization within the request.
5. Fees(for Parents):
   • Delaware: The fee is $0.25 per page and $5.00 for postage and handling if sent routine mail
   • Florida: The fee is $1.00 per page for the first 25 pages and $0.25 per page thereafter,
   • Pennsylvania: Process Fee $16.56
     • $1.11 per page for pages 1 - 20
     • $0.84 cents per page for pages 21 - 60
   • New Jersey: Process Fee $10.00 per request and:
     • $1.00 per page for pages 1 - 100
     • $0.25 cents per page for pages 101 - 500
