If you or your child have health coverage through Medicaid or the Children’s Health Insurance Program (CHIP), your state will be deciding over the next 12 months whether your family still qualifies for this coverage. Your care team at Nemours Children’s Health wants to make sure you have the information needed to make informed decisions.

Delaware

How do I update my personal information?

Contact your state’s Medicaid office to make sure they have your updated address, email, phone number and any recent name changes for you or a family member. Here are a few ways to do that:

  • Update your information online. In Delaware, visit the Division of Medicaid and Medical Assistance. Updating information online is preferred, as phone wait times can be long.
  • Update your information by phone. Call the Delaware Division of Health and Social Services. To update personal information, income or to add a newborn, press option 2. For renewals, case status or general questions, press option 1.
  • Fill out a form sent to you in the mail. Check your mail for a letter from your state Medicaid agency. This letter will let you know if you need to complete a form to update your information.

New Jersey

How do I update my personal information?

Contact your state’s Medicaid office to make sure they have your updated address, email, phone number and any recent name changes for you or a family member. Here are a few ways to do that:

  • Update your information online. In New Jersey, visit the web page Stay CoveredNJ. Updating information online is preferred, as phone wait times can be long.
  • Update your information by phone. Call the Department of Human Services (TTY: 711).
  • Fill out a form sent to you in the mail. Check your mail for a letter from your state Medicaid agency. This letter will let you know if you need to complete a form to update your information.

Pennsylvania

Should I apply for health care coverage renewal?

If you are eligible to renew your health care coverage, you’ll receive a renewal letter in the mail from your state Medicaid agency. Complete this renewal form and return it right away to help avoid a gap in Medicaid or CHIP coverage. Contact your state Medicaid office with any questions or concerns regarding your renewal.

What if I lose health care coverage?

A loss of health care coverage is concerning. You or your child still may be eligible for coverage through your employer or through the Health Insurance Marketplace. Call for details.
