Population Health Framework
As Nemours Children’s pursues our mission to create the healthiest generations of children, we are focusing on fostering the optimal developmental, physical, mental, behavioral, and social health of all children. We achieve this through high-quality health care, effective population health management, and high-impact population health initiatives.
To advance population health, we have developed the Population Health Strategic Framework to take the population health work we have embraced for years to the next level by shifting to a systemwide focus and partner-based approach. We seek to improve conditions in the communities we serve which, in turn, will improve health.
