Screening & Background Forms

Nemours Children’s Hospital, Delaware provides Graduate Medical Education training for trainees enrolled in residency and fellowship programs at other institutions. Requirements vary depending on whether or not your program is affiliated with us.

Requirements for Trainees From Affiliated Institutions

Nemours Children's is committed to providing quality care in a safe environment to those we serve. One method of ensuring this commitment is to maintain background pre-employment screening processes consistent with health care industry standards and in compliance with all applicable laws and regulations related to criminal background and abuse registry checks. Nemours will take responsibility for all expenses related to these requirements.

 

Required Forms

All forms may be emailed to GME@nemours.org.

If a visitor has been here before, please submit a GME Form. The GME office will inform the program on any needed items.

Note: Foreign medical graduates must have an updated validation sticker.

TJU and CCHS residents or fellows do not need to complete this form. Validation of immunization for TJU and CCHS takes place via a separate process but will require your signature on a release form.

Additional Requirements:

All institutions must submit a certification of malpractice covering the educational visitor.

Urine Drug Screening

A urine drug screening is required for all trainees.

Note: This test can be done at any Quest Laboratory as long as you have the Nemours form. TJU and ChristianaCare are exempt from this process.

Break in Training

Residents and fellows who have had a break in training must provide a curriculum vitae or a written and signed explanation.

Any time off between graduation from medical school and the beginning of residency is considered a break in training, whether for research, family matters, travel or other reason.
