Medically Complex Primary Care at Nemours Children's is now available in Jacksonville, offering accessible, local care for children in Northeast Florida with complex conditions.
In addition to Nemours pediatricians providing the full spectrum of primary care, the clinic will also give your family access to a dedicated care coordination team of nurses and social workers who can help you navigate the support your child needs to thrive.
If your child has a complex condition, or is dependent on medical equipment or technology, we’re here for you.
What to Do First
To be placed on the list for an appointment, please call us at (904) 697-3600. We’ll follow up to confirm your interest and ensure your family has everything you need ahead of an appointment.
In the meantime, you should request prescription refills, medical records, and school forms from your current provider to help prevent a gap in your child’s care as they transition to our clinic.

We provide primary care for kids with a wide variety of complex medical conditions, including (but not limited to):
Technology Dependence & Intensity of Care
- Tracheostomy and/or ventilator dependence
- Feeding tubes (G-tube, GJ-tube) and TPN or enteral nutrition
- Oxygen, CPAP, or continuous cardiorespiratory monitoring
Medically Fragile & Multisystem Conditions
- Complex congenital syndromes and chromosomal disorders
- Severe cerebral palsy with multisystem involvement
- Transplant-related conditions
Neurologic & Neuromuscular Conditions
- Spinal muscular atrophy (SMA)

Frequently Asked Questions
Due to the nature of the clinic, we require patients to be accepted before we can see them. Families may request a referral from their primary care provider or call us to self-refer. At this time, we are not accepting siblings without medical complexity.
We accept most major insurance plans and government programs. Before your visit, our team will be happy to help you understand your coverage and options.
Once your child is accepted to the clinic, a member of our team will reach out to you to begin the process of getting care transferred over. This may include an intake phone call with a care coordinator and a conversation with our scheduling team around insurance to ensure everything is in place prior to your first appointment.
Every appointment is different, as no two children are the same and we want to make sure we have the time to deep dive into the many things that make your child unique. Our well child visits last around 80 minutes on average, while new patient appointments are typically longer and may last a few hours.
Yes, your child can continue to see their specialists. We will simply be taking over primary care responsibilities from your current pediatrician.
