Care Management Program
When your child is ill, there’s a lot to keep track of and there may be many aspects to manage. It can be confusing and stressful.
- Does your child have multiple providers and/or multiple chronic conditions?
- Do you need help getting to appointments or getting appointments scheduled?
- Are you feeling overwhelmed?
- Would you like to learn more about resources in the community that could help you and your family manage your child’s health care needs?
Care management is a practice model that involves the patient/family, nurses, social workers and physicians. We partner with home care, insurance providers, caregivers and the community on behalf of our patients and families. Our care management team helps to facilitate communication, coordinate resources and referrals, and provide you the tools and guidance to help your child achieve optimal health.
What Can Care Management Do for Me?
The care management program was developed to support patients and families. Our goal is to help your child stay healthy, out of the emergency department and out of the hospital.
- Key point of contact — Registered nurse and social worker to contact for concerns, questions or support.
- Coordination of services — Care managers will assist with coordinating multiple appointments. They will stay in constant communication to eliminate duplication of service and provide support with a multitude of community resources.
- Individual care plan — Care managers work with the patient/family around health care needs. They will develop goals with the patient/family that will assist in the self-management of the patient’s condition.

The care management program is a free service provided to eligible patients. Please contact our care management program at (302) 298-7719 from 8 a.m. to 4:30 p.m. You can email our care managers at caremanagement@nemours.org for more information, or ask your health care provider.
