The Nemours Center for Advanced Care at Home provides specialized care to pediatric patients with unique medical needs, supporting their safe transition from hospital to home with 24/7 remote clinical oversight. Our program is designed for kids who no longer need inpatient care, but still require close medical supervision.
Our short-term care model provides patients with in-home monitoring, medical equipment, and direct access to our specialists — so you can transition patients home with confidence.

Our program helps reduce the risks associated with discharging complex pediatric patients, such as readmission, feeding issues, and caregiver burnout.
Our remote in-home services include:
- Daily weight monitoring
- Intake/output tracking and fluid balance support
- Feeding oversight and technique coaching
- NG and G-tube management
- Blood pressure monitoring
- Pulse oximetry and heart rate monitoring

- Palliative care coordination

Who Is Eligible
Our program is designed for kids who:
- Are transitioning from a hospital stay and need continued clinical oversight
- Are medically stable and ready to continue care at home
- Require ongoing monitoring, feeding support, or chronic care management
Referrals are made by our care teams.
Our Team
Jennifer Liedel, MD
Medical Director
Dr. Liedel is a board-certified critical care physician and has been providing expert critical care to children since 2004. She has conducted research on several topics in neonatology and has lectured regionally and nationally on topics such as feeding issues in the Neonatal Intensive Care Unit (NICU), breast milk and inflammation in newborns, and the use of extracorporeal membrane oxygenation (ECMO) in the NICU.
Christopher Beaty, MSN, RN
VP, Nursing/Operational Innovation
Christopher Beaty leads clinical innovation across our system. He has spearheaded strategic projects including a multi-phase NICU expansion, the establishment of an outpatient perinatal testing center and most recently the formulation of an operating plan for the nation’s first specialized support program for children transitioning from hospital to home.
Anastasia Castillo, MD, FAAP
Division of Inpatient Medicine/Hospitalist
Dr. Castillo is a board-certified pediatrician and researcher specializing in advanced critical care. Her expertise includes managing complex conditions and caring for children admitted to the hospital as a pediatric inpatient medicine specialist, as well as babies in the Newborn Nursery. Her research spans pediatric organ failure, traumatic brain injury, and severe respiratory illness.
Ashish Gupta, MD, FAAP, MBA, CPE
Neonatology
Dr. Gupta is a board-certified neonatologist who has been treating newborns and children at Nemours Children’s Hospital, Delaware and throughout Delaware Valley since 2014. In addition to expert clinical care, he is a recognized leader whose expertise includes quality improvement, program development, and implementation of evidence-based practices in neonatology and pediatrics. He speaks English and Hindi.
Collin Miller, DNP, CPNP-AC
Director of Nursing
Collin leads virtual care initiatives, leveraging his extensive experience and expertise in pediatrics. He has supported the development of the Clinical Pathways department and the launch of over 15 clinical pathways, as well as numerous improvement initiatives including trauma room redesign, electronic trauma documentation implementation, and triage nurse education.
Osei Owusu, MD
Critical Care
Dr. Owusu is a board-certified pediatric critical care physician who has been providing expert care to critically ill children with complex diseases since 2012. In addition to experience in clinical care and resident education, Dr. Owusu has special interests in sepsis, ventilator management, and traumatic brain injury. He is highly experienced in multidisciplinary collaboration.
Nicholas Slamon, MD
Critical Care
Dr. Slamon is a board-certified pediatric critical care physician and has been providing expert critical care to children since 2004. In addition to extensive clinical care and leadership experience, Dr. Slamon has been an invited lecturer, presenter, and expert interview subject for his expertise in telemedicine and pediatric safety.
