Coordinating medical care for a child with a complex medical condition can be challenging — and time-consuming. Nemours Children’s Hospital, Delaware recognizes that parents and guardians can use some help getting appointments with all the right specialists at their preferred location(s). That’s why we’ve created the patient navigation service. We’re here to help you get the care you need and want — when and where you need and want it.
Our Patient Navigators
Our patient navigation team consists of nurses and designated schedulers to help families of children with complex medical conditions make all the appointments they need for a given visit to Nemours Children’s Hospital, Delaware — all on the same day, if possible.
If your child’s diagnosis will require specialist treatment across many different medical disciplines, our patient navigators can help. It’s their job to help families through the uncertainty surrounding a complex medical condition. We want to make sure you get appointments with the right physicians and providers for your child.
Common Conditions
Patient navigators can help schedule appointments for children with conditions such as, but not limited to:
- arthrogryposis
- bone tumors
- brain tumors
- cleft and craniofacial anomalies
- cerebral palsy
- concussions
- epilepsy
- hydronephrosis
- nephrology (kidney) conditions
- perinatal testing & specialty consultations
- skeletal dysplasia
- sleep apnea and other sleep problems
- solid organ transplant-related diseases (kidney transplant and liver transplant)
- spine deformities and scoliosis
Scheduling Appointments
To schedule your appointments using the patient navigation service, simply email patientnavigation@nemours.org. Our designated scheduler will help you make all of your appointments where and when you need them.
